Defence sorry for Afghan crash report delay

The Defence Force has apologised for the delay in releasing a report on the deaths of three Australian commandos in a helicopter crash in Afghanistan last year.

The report found the crash was not caused by enemy fire or mechanical failure, but by a lack of air crew coordination during the approach to the landing zone that led to a loss of situational awareness.

It found Privates Tim Aplin and Scott Palmer died instantly, while Private Ben Chuck died of his injuries on arrival at a medical facility in Kandahar.
